2011 CalPERS Open Enrollment
2011 CalPERS Open Enrollment
Dates:    Monday, October 10, 2011 through Friday, November 4, 2011.  Changes to be effective January 1, 2012.

IMPORTANT:  The Open Enrollment periods for other benefit programs such as dental plans, Health Care Spending Account, Dependent Care Assistance Program and the Premium Conversion Plan are scheduled for Monday, October 31, 2011 through Friday, November 18, 2011.  This information is scheduled to be mailed the week of October 24, 2011.


ACTIVE EMPLOYEE
Be sure to read the information from the County as it includes information that supplements the correspondence received from CalPERS.  Information from the County includes a Benefit Statement listing the CalPERS health plan and other benefits you receive as an employee of Contra Costa County.  Be sure to compare health plan information on this benefit statement to the benefit statement you received from CalPERS.

If making changes, please submit the Open Enrollment change form to Employee Benefits no later than 5:00 p.m., November 4, 2011.  Change form is two pages, you are required to initial and date the first page in addition to completing sections 1 through 21 on the form.  FORM IS AVAILABLE FOR USE BY ACTIVE EMPLOYEES ONLY, RETIREES MUST MAKE CHANGES DIRECTLY WITH CALPERS.

RETIREE/SURVIVOR
Retirees and Survivors must contact CalPERS directly to make any Open Enrollment changes.  Contra Costa County requests that our office be notified of any changes to insure that our records are current and that the correct deduction is made from retiree pension check.  Please call (925) 335-1746 with any changes.

Dental plan open enrollment is scheduled for October 31, 2011 through November 18, 2011.  Information will be sent the week of October 24, 2011.
