Second Complaint - Noisy Animal

If you have completed the Noisy Animal Complaint Form, we will be sending both you and the animal's owner/caretaker letters about the complaint on the next business day (business days for CCAS are Tuesday through Saturday). The owner/caretaker will have 15 days from the date on the letter to correct the issue. If the issue is not corrected in that time, you can then complete the second complaint process to move this to a citation/hearing.

PLEASE NOTE: If you have not submitted a first complaint, you must do so before completing this form. Persons who submit a first complaint will have received an activity number, which is required to complete this form. If you do not have an activity number stemming from a first complaint, this form will not be accepted.

Submitting This Form: You have two options to submit this affidavit:

  1. Download and print the form, fill it out and bring to Contra Costa Animal Services’ Martinez or Pinole offices Tuesday-Sunday10:00am-5:00pm (10:00am-7:00pm on Wednesdays) to be witnessed by a designated CCAS employee. Our office locations are as follows:

  2. Download and print the form, fill it out and have it notarized. Send the notarized form to our Martinez office. Be sure to identify on the envelope “Animal Noise.”
